A Day in K
If your child will be 5 years old prior to August 1st, 2012 please call 256-6158 to RSVP for a “Day in K” on April 13th or April 20th from 9:30am to 11:00 at the West Plains Elementary. Parents or guardians must bring a birth certificate for the child, immunization record, Social Security number, proof of residency, information about allergies or special conditions, physician’s phone number, and court order (if applicable) for the child entering preschool or kindergarten.
Parents and guardians may fill out some of the necessary paperwork before a "Day in K", please click on the links below. Once each form is completed please click on SUBMIT at the top left hand corner. All forms will printed at West Plains Elementary and ready for a signature when you arrive.
