Physical Therapy Specialists Clinic administers the wellness program for the R-7 Schools. PTSC has modified the program around the school year calendar to meet the needs of the faculty and staff. The program begins September 1, 2010 and ends May 31, 2011. Sign up by August 31st and receive a wellness discount card, good for discounts at local retailers!
How to enroll:
Click on the "document manager" tab on the left and download an enrollment and consent form for you and your family members that are joining.
Print and complete the enrollment and consent forms by AUGUST 31.
Turn in your paperwork to Becca Frazier at Central Admin.
Note: The payroll deduction option will begin in September - deadline to sign up is 8/31.
Call PTSC and sign up for a time for your initial testing. You can test on September 2nd or 3rd between 7:00-8:30am at the clinic.
Wear or bring shorts and tennis shoes for initial testing. Blood work will be drawn so you should not eat or drink for 8 hours prior.
